RADICI WAIVERS, TERMS, CONDITIONS:
COVID-19 TRAVEL WAIVER FORM
As the worldwide COVID-19 corona-virus pandemic remains ongoing at this time, I acknowledge that for this reason, and other reasons not reasonably foreseeable at this time, these travel plans may be interrupted or canceled by the supplier that is providing forms, a government entity or other third party over which Radici Siciliane has no control. I further acknowledge that the supplier's own cancellation, re-booking and refund policies, subject to any applicable law that is now or may later be in effect, will govern my rights and remedies, including my right to receive a refund, in such an event. Moreover, I understand that should I elect to purchase travel insurance, the terms of the policy will dictate whether, and to what extent, coverage for any financial loss may exist under the circumstances. By signing below, I hereby agree to hold Radici Siciliane harmless and release the agency from any and all liability for any damages, including but not limited to monetary losses, I may incur as a result of such interruption or cancellation of these travel plans.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while traveling. Such exposure or infection may result in personal injury, illness, permanent disability, and possible death. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with Radici Siciliane.
As travel opens around the world, all destinations, airports, air carriers, hotels, restaurants, transfer companies, car rental companies, shops and excursions have established COVID-19 safety measures and precautions which may change from day to day. These safety measures may include, but are not limited to: curfews, attraction closings and reduced hours, size of group gatherings, social distancing requirements, health screenings, self-quarantine requirements, COVID test results, and vaccinations. By signing this agreement, I accept ultimate responsibility for myself and my traveling party to have all the necessary provisions for travel (such as COVID test results, pre-travel questionnaires, vaccination cards etc.) Moreover, I understand that I should assume responsibility for the necessary documents (such as COVID test results, pre-travel questionnaires, vaccination cards etc.) considering COVID-19, in order to travel to my specific destination.
AGENCY –
RADICI SICILIANE is acting as THE ORGANIZER and is not the direct source or supplier of the experiences in Sicily. Radici Siciliane works directly with suppliers in Sicily, but is not fully responsible for any decisions or changes made by the suppliers. Traveler agrees not to hold Radici Siciliane responsible should any of these suppliers: 1) fail to provide the travel services purchased; 2) fail to comply with any applicable law; or 3) engage in any negligent act or omission causing injury, damage, delay or inconvenience.
Radici Siciliane is not responsible for, nor can it be held liable for, any injury, damage or loss suffered because of any conditions, actions or omissions that are beyond its reasonable control.
Travel to many parts of the world may involve exposure to a variety of health and/or safety hazards, including but not limited to disease, crime, terrorism and warfare. Radici Siciliane is not responsible for advising travelers of whether they should travel to any particular place at any particular time. It is the traveler’s responsibility to stay informed by referring to objective third-party sources of travel information, such as that maintained by the U.S. Department of State (travel.state.gov).
PROOF OF IDENTITY –
The Transportation Security Administration (TSA) requires passengers to carry a government issued identification (ID) card in order to board a flight. The name, date of birth and gender that appears on the traveler’s ID card must exactly match the same such information that is listed on the airline ticket and in the booking records.
International travel requires travelers to have a valid US Passport book.
Travelers under the age of 18 not traveling with both parents may need additional special or legal documentation to travel.
It is the traveler’s responsibility to be aware of, obtain, and provide the proper documentation required for their trip. Failure to strictly comply with these requirements may result in denied boarding, undue delays, and/or fees resulting in additional cost to the traveler(s). Radici Siciliane is not responsible for these fees and no refunds will be made.
Passport and visa information may be obtained by contacting the U.S. State Travel Advisory Department at (202) 647-5225 or by visiting the State Department's Web site at travel.state.gov. Non-U.S. passport holders should contact the embassies of their destination and transit countries to obtain entrance requirements. To obtain medical information, please contact the Centers for Disease Control at (404)332-4559 or visit the CDC's website at www.cdc.gov.
RATES & INCREASES –
All quoted rates are subject to change without notice until initial deposit or payment has been made.
Airline tickets, air tours and/or other products may be subject to supplemental price increases after the date of purchase due to additional costs imposed by a supplier or government agency. This may include, but is not limited to, increased fees, fuel surcharges, taxes, fluctuations in foreign exchange markets or any combination thereof.
Radici Siciliane strongly suggests using a credit card for purchasing plane tickets in case of cancellation; it is much easier to reverse the charges than with a debit card.
TRAVEL INSURANCE –
Radici Siciliane strongly recommends travel insurance be purchased for all travelers at the time of initial deposit to cover risks inherent in travel. Radici Siciliane and their Advisors are not insurance experts and cannot guarantee the adequacy or extent of an insurance policy. Travelers are advised to read and become acquainted with policy documents and contact the insurer for clarification or concerns.
If you choose not to get travel insurance, you will need to fill out the waiver below.
RADICI SICILIANE INSURANCE WAIVER FORM
I am refusing travel insurance for my trip.
The risks for declining coverage have been explained to me by my Travel Advisor. I understand that declining travel insurance mean I will lose all benefits of coverage and I fully accept that risk.
I am aware that my own medical insurance may not cover me outside of the United States.
I UNDERSTAND THAT I WILL NOT BE COVERED FOR:
Lost luggage
Missed connections or train delays
Emergency air lift assistance
Emergency medical and/or dental assistance
Emergencies at home affecting me and/or my immediate family (children, grandchildren, aunts, uncles, brothers, sisters, nephews, nieces)
Death of myself and/or my immediate family (children, grandchildren, aunts, uncles, brothers, sisters, nephews, nieces)
Supplier default and/or bankruptcy
Any other covered circumstances as outlined in the offered travel insurance policy
COVID-19 related travel restrictions or cancellations
I understand that I may lose up to 100% of the cost of my trip, plus any cancellation fees from Radici Siciliane, if my trip is cancelled or delayed.
I hereby release Radici Siciliane from responsibility for any consequences, both known and unknown, resulting from my refusal of trip cancellation insurance.
I understand that Radici Siciliane has advised me of the importance of such insurance and I will not hold them responsible for any risks or lost funds. By signing this form, I confirm that I do not expect Radici Siciliane to assist me in any way if my trip is cancelled or delayed for any covered reason.
PAYMENT AGREEMENT:
I agree to paying a $900 deposit to save my spot. $500 of this deposit is non-refundable. If I choose to cancel before January 1, 2022, I will be refunded my $400. After January 1, 2022, Radici Siciliane will keep the entirety of the deposit. After April 1, 2022, there will be no refunds of the total amount you have paid to that date. ITALIAN TRAVEL REQUIREMENTS AS OF September 1, 2021 and will stay this way until October 25th 2021. That does not intersect with our trip, but it could extend into our trip.
Any traveler who has been in the U.S. in the previous two weeks must present a negative coronavirus test taken within three days of their arrival in Italy regardless of their vaccination status, the Italian Ministry of Health said Tuesday.
Unvaccinated travelers must also quarantine for five days after they arrive and then be tested for coronavirus again, even if their initial test was negative.
Tourists must also fill out a passenger locator form, which allows contact tracers to identify viral clusters in the event of a coronavirus breakout.
The new restrictions also apply to travelers coming from Japan, Canada and Israel, which are also battling new coronavirus waves of their own.
Italy’s tougher requirements will last until at least Oct. 25, the government said Tuesday.
What Radici Siciliane suggests is that you- beyond the travel requirements of the time- is to:
1. Have proof of natural immunity to COVID and/or 2. Show proof of a vaccination.
We will be traveling through small villages and visiting with many elders. It’s our job to keep them - and ourselves- safe. By having natural immunity and being vaccinated we can ensure that.
I agree to comply with whatever Radici Siciliane and the Sicilian travel requirements are for travel when the date of travel approaches, including but not limited to, being tested, vaccinated and/or having natural immunity.
